1. Information we collect
We collect only the information reasonably needed to provide safe care and operate the practice. This may include:
- Your name, date of birth, contact details, emergency contact, address and billing information.
- Medical history, allergies, medicines, previous procedures, contraindications, consent, clinical notes and follow-up records.
- Clinical photographs or skin images used for assessment, treatment planning and documentation.
- Appointment, communication, payment and transaction information. We do not store complete payment-card details.
2. How we use information
We use information to assess suitability, provide and document care, monitor outcomes, communicate about appointments and aftercare, manage accounts, improve quality and meet legal, professional, insurance, safety or reporting obligations. Marketing use requires separate, explicit consent.
3. Storage and retention
Records are held in access-controlled clinical systems and secure storage. Authorised people may access them only for legitimate purposes. Health records are retained for the period required by New Zealand law and professional standards—commonly at least 10 years from the last encounter—then securely destroyed or anonymised when appropriate.
4. Disclosure
We do not sell or rent your information. We disclose it only with your authority, for safe referral or continuity of care, to service providers operating under appropriate safeguards, where law or a regulator requires it, or where permitted to prevent or lessen a serious threat to health or safety.
5. Your rights
Subject to lawful limits, you may:
- Ask for access to your personal and health information.
- Request correction and have a statement of correction attached if a correction is not made.
- Withdraw optional photography or marketing consent.
- Ask how information is used and complain without disadvantage to your care.
6. Photographs
Clinical photographs form part of your confidential health record when needed for assessment, safety or progress documentation. Publication, social-media, education or marketing use requires separate written consent describing the intended use. You may decline marketing use or later withdraw consent, although material already lawfully published may not be fully retrievable.
